By completing the tooth damage form, you will ensure that the dental accident sustained by your patient and our policyholder is processed quickly.
Please complete the "Tooth Damage: Findings/Cost Estimate" form in all cases and submit it to us together with the relevant documentation for your patient. The claim for tooth damage will be notified to us only when this tooth damage form is submitted, and any consequential damage can then be assessed.
Please send the patient's documentation together with the tooth damage form to the address on the form.
